Compassionate Home Care Team

Compassionate Home Care is different from other home health care agencies. We strive to go the extra mile, to assure the patient and the patient’s family that their needs are our most important priority.

Our Team Includes:

• A nurse to visit the patient according to the patient’s Plan of Care. The nurse also instructs caregivers to care for the patient in the home.
• A physical, occupational, and/or speech therapist to assist with rehabilitative needs.
• A social worker to assist the patient and family with understanding the emotional factors related to a medical diagnosis. He/she also assists in facilitating the discharge process by assisting the patient and family in locating outside community resources.
• A home health aide to provide personal care, light housekeeping, and prepare nutritious meals.
